It would be gratifying to show a body of research that answers the 
questions asked:

But so far, I have not discovered it.

What I hoped was that among the many communities who received these 
questions I would have heard back about some series of studies,  or a 
comprehensive literature review, or a systematic meta-analysis that 
we could all examine and discuss. Did not happen.

Has there never been a published literature review on these 
questions?  Is this something that is done by a different discipline 
from ours?

There were responses, but not research-based evidence, or county or 
state data that address these questions.

Maybe I missed something?

I have not heard back that USA is going to (or already did) publish a 
report. I do appreciate the few responses I received from this and 
many other lists.

If I have misinterpreted anyone's response to me, it is my mistake. 
Please share your research-based evidence with this list.
