Dr. Daniels, I'm not sure that it qualifies as a "research instrument", but The Sentinel Questionnaire Follow-Up Study, an independent policy-relevant substudy of The Third National Incidence Study on Child Abuse and Neglect (NIS-3), contains the survey questionnaire sent to all school personnel who participated in the Basic NIS Sentinel Study as sentinels in schools, as well as the findings from the Study. The survey questions covered such aspects as personal experiences in reporting and attempting to report cases to CPS and the factors that influenced the decision to report or not to report. The data were gathered to provide information concerning the extent and types of barriers to reporting suspected maltreated children to local CPS. Copies of this report are available through the National Clearinghouse on Child Abuse and Neglect Information: publication number 20-10215 (cost recovery fee of $17.00). Please contact the Clearinghouse at: nccanch@xxxxxxxxx or by phone at: (800) 394-3366. Thank you, Sandi McLeod National Clearinghouse on Child Abuse & Neglect Information (800) 394-3366, ext. 352
