Massachusetts has a statewide program for first time teen mothers based on the Healthy Families approach. It is, in fact, called Healthy Families Newborn Home Visiting. I am happy to put you in touch with the appropriate people if you'd like to send me a personal message (I would be the person to talk to about the evaluation of this program). I believe Indiana's similar program has gone statewide as well, but think that it is for families at high risk for child abuse and neglect only.
