substantiation levels

levels of substantiation that different jurisdictions use in child maltreatment
investigations. We would like to know which jurisdiction use two
levels of substantiation (confirmed - not confirmed) and which use three or more
(definitive levels and a suspected level). We would like information for
Canadian, American and other jurisdictions. If you can provide this
