Good questions, I would suggest looking at the YRBSS,
Youth Risk Behavior Surveillance Survey for measures
of risk-taking behaviors, as far as psychological
well-being Mirowsky and Ross discuss a number of
meaures of well-being and it would be a great starting
place for references on measuring psychological
well-being and what "well-being" is. A survey that
uses psychological well-being meaures is the Detroit
Area Study, 1995 (DAS) available through ICPSR housed
at U. of Michigan. Hope this helps some. I do have
some measures/literature related to the measures in
the DAS, contact me if you would like that information
forwarded to you.
In regards to assessing a clinical survey--what do you
mean?
