instruments designed to measure characteristics of fosterparents

We are in the process of developing an on-line questionnaire as part of
a relatively large study of placement instability in substitute care.
Although, for logistical reasons, the survey respondents will be case
managers, the main "subject" of the survey will be relative and
non-relative foster care providers.

We are looking for any suggestions that people might have about
instruments designed to measure characteristics of foster parents or
other substitute caregivers (e.g. general adequacy, warmth/affection,
cooperation with case managers, etc.). Any and all suggestions would be
much appreciated.
