The final few minutes of Last Exit are hard to take... More depressing
(because they are realistic) movies include The Sweet Hearafter (bus
accident trauma, incest) and Requiem For a Dream (drug addiction).

Affliction is on my all-time most depressing movie list.  I believe it
occupies the #2 spot with Last Exit to Brooklyn coming in at #1.
Affliction doesn't have any foster care themes, though....
