RE: Child Fatality Review

In NSW, Australia, child fatalities are reviewed by the NSW Commission for
Children and Young People.  They have a Child Death Review Team.  Here is a

NSW Department of Community Services (CPS)  cooperates with the CDRT in
these reviews.
