Re: child in care deaths. Q. mandatory investigations?



My colleagues and I will be publishing a survey  of child deaths in 
institutional care ( 50 (US) state child welfare, mental health, 
juvenile justice, and mental retardation systems) as they relate to 
physical intervention and restraints.  Our survey "Learning from 
Tragedy " was reported in our newsletter REFOCUS vol. 6 
(rccp.cornell.edu), reported at Trauma 2002 (WHO) in Montreal this 
May.  In addition to learning a little about how children died in 
restraints, we uncovered a number of system problems throughout the 
50 states:  lack of central and consistent reporting in state, lack 
of a mandated public fatality review that includes root cause 
analysis, and mandated corrective actions to system's problems, and 
over dependence on criminal investigations.   We should have the 
survey results ready for journal publication by the end of the year.
