The recent UK Home Office evaluation of management and treatment is at

Be warned, it takes a long time to download (180 or so pages)
The lead author is Anthony Beech whose e-mail is A.R.Beech@Bham.ac.uk
I think you may be more interest in the day to day management than the
treatment but this may throw some light on it - I havent had time to read it
all.
