We have such a form in Arkansas.  The purpose is to determine if a person
has a founded case of child abuse/neglect on the central registry in our
State.  Many agencies, businesses and schools use the form.  You may get a
copy by contacting Pat Page at the Dept. of Human Services in Little Rock,
phone 501-682-8544 or at the email address above.
