Karen, I would strongly recommend Valerie Sinason's work.  She is based at 
the Tavistock Clinic in London and has done a great deal of work with 
children with disabilities who have been abused.  Much of her experience is 
with children and young adults who have difficulties in communicating their 
experiences and feelings.  I am sorry I cannot give you any exact references 
'off the top of my head' but I am sure you will be able to find her in a 
search.  Her perspective is grounded in psychoanalysis regarding actual 
treatment/therapy.
