Systems of caring

I am searching for how other agencies, whether state or private, work with 
families who have lost custody of their children to foster care. What tools 
or strategies do people use to reunify families? Also any programs that look 
at issues surrounding children in care, services that they need, something 
that follows them with medical records, mental heath record? What is out 
there? What are people doing? I would really appreciate any info, web sites, 
or ideas to start searching.
