RE: Early at risk interventions for adolescents
I am not sure exactly what "at risk" group you're targeting, but there's a
well developed and researched prevention program out of Rochester N.Y.
that's been very popular.
Below is a reference that'll help familiarize you with the program.

School-based prevention for children at risk: The Primary
Mental Health Project (1996).
(from the publicity materials) This book describes the
evolution and daily operation of the Primary Mental Health
Project [PMHP], an innovative school-based program [for
children at risk] that provides a viable alternative to
traditional "after-the-fact" intervention. [The book] provides
nuts-and-bolts descriptions of how to establish, maintain, and
evaluate such a program, as well as research evidence
documenting PMHP's efficacy.
