Washington State's Office of Children's Administration Research (OCAR)
has done two surveys: 

Community Survey: A community survey to obtain input from various
community stakeholders was developed and administered to examine
community perceptions regarding the perceived operation of DCFS. 

Client Survey: A client survey was developed and administered to obtain
client input regarding client's perceptions of the  operation of various
(aspects of) the CPS program.

I currently am supervising a research student who is 
examining clients perceptions of services received at a 
specialist theraputic unit and comparing clients 
experiences of this project to thier previous experiences 
of the child protection system. Would anyone have any 
particularly useful references or contacts on the subject 
of client evaluation of child protection agencies.
