1.       North Carolina Family Assessment Scale (NCFAS)

2.       The Strengths and Stressors Tracking Device (SSTD)

3.       The Family Assessment Form (FAF)

4.       The Family Assessment Checklist (FAC)

5.       Darlington Family Assessment System (DFAS)

Subject: In-home service outcome measures

 

I am looking for evidenced based practice models or outcome measures for in-home service programs - ours are called "Safe at Home" and "Families in Transition".  We basically send 2 therapists into the home to work with the family with the focus on keeping the child in the home, school and community.  Most of the referrals are the last chance to keep the kid out of foster care, residential care or other alternative placements.  Appreciate your thoughts on this.
