Ordering Instructions for the
NSCAW General Release Data
Users will receive NSCAW I GR (version 5.4.1) and
NSCAW II GR (version 2.0)
 

Please supply the following items:

1. Certification that your institution's IRB is registered with the U.S. Office for Human Research Protections (OHRP) and appears in the OHRP database.

2. A Web link to your institution's policies and procedures on scientific integrity and misconduct, including the name and address of the person or office responsible for enforcing them.

3. Certification that your IRB has either approved your project or has found it to be exempt from review

4. Three copies of the General Release Data License (PDF) for each institution involved in the proposed project with original signatures on all.

5. Completed and original signature copies (no faxes or photocopies) of the Supplemental Agreement with Research Staff (PDF) for each member of the research team other than the Investigator who will have access to the data. Research staff must be formally affiliated with the Investigator's institution (student, staff, faculty, etc.).

The General Release Data License (PDF) is a legal document between the researcher, the Receiving Institution, and NDACAN. Changes in the employment status of the researcher require the completion of a new Data License.

NDACAN will have full discretion in deciding whether the Investigator meets eligibility criteria. If NDACAN decides all requirements are met, a representative from NDACAN will sign the General Release Data License (PDF) and return a copy of the fully executed license to the applicant along with a copy of the data.

During the course of the research project, the investigator may apply to have research staff access the NSCAW data by submitting one Supplemental Agreement with Research Staff (PDF) for each staff person. The investigator cannot provided data access to these new research staff until the Archive countersigns Supplemental Agreement and returns a copy to the Investigator.
